In exchange for participation in the interactive activities organized by CallMiner, Inc. (“CallMiner”) and use of Two Bedroom Banana Republics Suite’s property, facilities, and services.

I agree to the following:

1. I recognize that there are certain significant risks associated with the interactive activities organized by CallMiner. These risks include, but are not limited to, serious bodily injuries and/or psychological harm caused by involvement in a stressful environment that may require some physical exertion. I agree to assume full responsibility for any personal injury I may sustain in connection with participation in interactive entertainment activities organized by CallMiner, and I release and discharge CallMiner and its owners, employees, representatives, and agents from any liability for any injuries I may sustain, including injuries caused by the negligence of CallMiner or its employees, representatives, or agents.
2. I certify that I am physically fit for participation in Two Bedroom Banana Republics Suite and I certify that there are no health-related reasons or problems which preclude my participation in Two Bedroom Banana Republics Suite’s experience.
3. I agree to pay for all damages to the facilities of Two Bedroom Banana Republics Suite caused by my negligent, reckless, or willful conduct.
4. I agree to observe and obey all posted rules and warnings, and to follow any oral instructions or directions given by Two Bedroom Banana Republics Suite or its employees, representatives or agents.
5. I acknowledge that this Release of Liability and Assumption of Risk will be used and relied upon by the released parties, and that it will govern my actions and responsibilities at Two Bedroom Banana Republics Suite. I acknowledge that management reserves the right to ask me to leave at anytime.
